The Ohio Department of Aging has modernized the SFMNP starting for the 2024 season. Eligible Ohioans now have the choice to use a mobile application or a physical card to obtain nutritious produce at farmers markets. ODA has partnered with Homegrown Benefits to build and maintain the new SFMNP app which can be downloaded on Google Play Store and Apple Store.
For older adults interested in applying for the program, click here for the online SFMNP application form. The online application form is the quickest and most efficient way to apply for the SFMNP program. An email address is required to fill out the electronic application, as application approvals will come via email from the Ohio Department of Aging.
If you do not have an email address, smart phone or the ability to complete an online application, you can reach out to the Agency for a paper application to receive a physical benefit card. At this moment, we have not received the paper applications. Please be aware that this application process will take 4-6 weeks to receive your coupons.
