Family Connects is a home visiting program for families with newborns, designed to provide personalized support during the crucial early weeks of life. Our dedicated team of nurses will be there to offer guidance, answer questions, and ensure the well-being of both parents and their newborns around three weeks postpartum. This program not only enhances health outcomes but also fosters a sense of community and empowers families to confidently navigate the joys and challenges of parenthood. We are committed to giving every baby the best possible start in life.”

The first few days and weeks home with a newborn can be a challenging time for everyone. Services include a baby health check, Mom’s health check, post-partum depression screening, community connections, and more.
