I Am Well Foundation is looking for a Whole Person Care Coordination Navigator to join their team!
This part-time, hybrid contract position offers the opportunity to make a meaningful impact by helping high-risk individuals connect with resources, navigate systems, coordinate care, and access services that support their overall health and well-being.
Position: Whole Person Care Coordination Navigator
- Hours: 20–25 hours/week
- Contract Rate: $30,000
- Work Setting: Hybrid — remote, on-site and community outreach
- Education: Associate degree preferred; background in social work, counseling, healthcare or another helping field is a plus
The ideal candidate will have experience with care coordination, working with vulnerable populations, volunteer or partner coordination, problem-solving, and connecting individuals with resources.
